Our Goal is to provide the highest quality of care possible to our patients. We do this by listening to and understanding your needs.

We are located in the Centerstone Plaza on the corner of Barranca Parkway and Culver Drive across from Mimi's Cafe. For directions, see the map to our office.

We value your time and that of your fellow customers. We require at least 24 hours notice if you need to reschedule your appointment in order to avoid a $25 rescheduling fee.

What to Bring

You want the best care and prescription for your eyes and your lifestyle. Please bring the following to your exam so that we can understand your vision history and be most efficient with your valuable time.

• current glasses AND sunglasses
• current contact lens information
• any previous eye exam records you may have
• completed Patient History Questionnaire
• completed Computer Vision Questionnaire if you use a computer at work or home
• completed Dry Eye Questionnaire if you have problems with eye dryness
• vision care plan or medical insurance ID information

What to Expect

During your first visit with us we will spend a little extra time in reviewing your information so that we can fully understand your specific eye health and support your specific needs. Our practice is fully HIPAA compliant and we have already adopted Electronic Medical Records for all of our patients. As part of ensuring your record security we do ask to take a photo of you for our files as an additional security step to ensure that unauthorized people don't get access to your private health information.

At your convenience, please print, fill out and bring the Patient History Questionnaire with you on your first office visit.

Computer Vision Questionnaire
If you use a computer more than 2 hours a day, please read and complete our Computer Vision Questionnaire.

Dry Eye Questionnaire
Complete our Dry Eye Questionnaire if you think you have problems with dryness to your eyes. Please bring the form with you to your next visit to Woodbridge Optometry. The doctors will diagnosis the type of dry eye and prescribe a treatment plan to help control your dry eye.
