We value your time and that of your fellow customers. We require at least 24 hours notice if you need to reschedule your appointment in order to avoid a $40 rescheduling fee.

You want the best care and prescription for your eyes and your lifestyle. Please have the following with you for your exam so that we can understand your vision history and be most efficient with your valuable time.

What to Expect

During your first visit with us we will spend a little extra time in reviewing your information so that we can fully understand your specific eye health and support your specific needs. Our practice is fully HIPAA compliant and we have already adopted Electronic Medical Records for all of our patients. As part of ensuring your record security we do ask to take a photo of you for our files as an additional security step to ensure that unauthorized people don't get access to your private health information. While many doctors are still thinking about transitioning to EMR, we stay ahead with new technology at Woodbridge Optometry.

Patient History Questionnaire
At your convenience, please print, fill out and bring the Patient History Questionnaire with you on your first office visit.

Computer Vision Questionnaire
If you use a computer more than 2 hours a day, please read and complete our Computer Vision Questionnaire.

Dry Eye Questionnaire
Complete our Dry Eye Questionnaire if you think you have problems with dryness to your eyes. Please bring the form with you to your next visit to Woodbridge Optometry. The doctors will diagnosis the type of dry eye and prescribe a treatment plan to help control your dry eye.

Financial Policy
If you are a new patient with us or if your information has changed, please read, print, and sign our Financial Policy agreement and bring this with you on your appointment.
